Before you publish

This is a draft website privacy policy for the Restored Life websites. Fill in the bracketed items, confirm it matches what your sites actually collect, and have your compliance lead or attorney review it before it goes live. It is not legal advice.

  1. In Duda, add a page named Privacy Policy (URL: /privacy-policy ) on each website, and link it in every site's footer.
  2. All placeholders are filled in. Update the effective date if you publish on a different day.
  3. Check what each site really collects: contact or admissions forms, chat widgets, newsletter signups, analytics, cookies, and any Facebook Pixel or Google tag.
  4. Copy everything from the "Privacy Policy" heading down into a text widget on that page.
  5. Use the published page's address for both Meta fields: Privacy Policy URL and User data deletion. The "Requesting deletion of your information" section covers Meta's data deletion requirement.

One caution: tracking tools like the Facebook Pixel or Google Analytics on pages about treatment, admissions, or insurance can send health-related information to those companies. HHS has warned healthcare providers about this. Ask whoever manages the sites whether any are installed, and keep them off those pages unless your compliance lead approves.

Privacy Policy

This Privacy Policy explains how Restored Life Continuum of Care and its affiliated organizations, including Wabash Recovery, Restored Life Health Network, and Restored Life Foundation ("Restored Life," "we," "us"), collect, use, and protect information through our websites, social media pages, and online tools.

It applies to the websites Restored Life operates and to our pages on Facebook, Instagram, and Google. It does not apply to other companies' websites, even if we link to them.

Your health information

If you receive services from us, your health information is protected by federal law, including HIPAA and the federal confidentiality rules for substance use disorder records (42 CFR Part 2). How we use and protect that information is described in our Notice of Privacy Practices, which you can request at any of our locations or by calling 765-780-7689. If this policy and the Notice of Privacy Practices ever conflict about health information, the Notice of Privacy Practices controls.

Please do not share personal health details in public places such as comments or reviews on our social media pages. To talk privately, call us or send a direct message.

Information we collect

Information you give us. When you fill out a contact, admissions, volunteer, or donation form, use the chat feature on our website, sign up for updates, or email or message us, we collect what you choose to share. This may include your name, phone number, email address, and your message.

Information collected automatically. Like most websites, ours collect basic technical information when you visit, such as your browser type, device type, the pages you view, and the general location your internet connection indicates. This comes from cookies and similar tools, described below.

Information from social media. When you follow, comment on, message, or review our Facebook, Instagram, or Google pages, those platforms share with us what their settings allow, such as your public profile name and what you post. Their own privacy policies govern what they collect.

How we use information

We use the information we collect to:

  • Respond to your questions and requests, including admissions inquiries
  • Share updates, events, and resources you asked to receive
  • Process donations and volunteer sign-ups
  • Understand how our websites are used so we can improve them
  • Keep our websites and pages secure
  • Meet legal and regulatory requirements

We do not sell your personal information.

How we share information

We share information only as needed to operate our websites and services:

  • Service providers that help us run our websites, forms, website chat, email, and donations, such as our website platform, Duda. They may use the information only to provide services to us.
  • Legal requirements, when the law requires it or to protect the safety of any person.

Health information from people receiving services is shared only as allowed by our Notice of Privacy Practices and federal law.

Cookies and analytics

Cookies are small files a website stores on your device. We use them to make our websites work and to see, in general terms, how visitors use them. This includes analytics built into our website platform, which count visits and show which pages are most useful.

You can block or delete cookies in your browser settings. Some parts of our websites may not work properly without them.

Social media and our apps

We manage our Facebook and Instagram pages through tools we build using Meta's platform. These tools publish our own posts and read public engagement with them, such as reaction and comment counts. They do not collect personal information from people who view or interact with our pages beyond what Meta makes available to page owners.

Security

We use reasonable administrative, technical, and physical safeguards to protect information. No website or internet transmission is completely secure, so please don't send sensitive health details through website forms, email, or social media messages. Call us instead.

Children

Our websites are not directed to children under 13, and we do not knowingly collect their personal information online. If you believe a child has sent us information, contact us and we will delete it.

How long we keep information

We keep information from our websites and social media only as long as needed for the purposes above or as the law requires.

Your choices

  • Emails and texts: use the unsubscribe link in any email, or reply STOP to any text message.
  • Cookies: adjust your browser settings.
  • Social media: manage what you share through your Facebook, Instagram, or Google settings.

Requesting deletion of your information

You can ask us to delete personal information we collected through our websites, social media pages, or apps. To make a request:

  • Call 765-780-7689 and ask to make a data deletion request, or
  • Write to us at 302 S Reed Rd, Kokomo, IN 46901, with "Data deletion request" at the top of your letter

Tell us your name and how you contacted us (for example, the website form or Facebook) so we can find your information. We will confirm your request and complete it within 30 days, except for information the law requires us to keep, such as health records.
